Ten people in an affected neighborhood know ten different pieces of what
happened. This card is how one of those pieces gets written down clearly
enough to be useful to whoever’s compiling the picture.
The front carries the exact structure taught in PLN-03: Community SITREP —
name and callsign, how to reach you, date and time, an update number,
location and coordinates, what happened, who’s affected, and a status/trend
table for all seven Community Lifelines, in the correct order. Ruled lines
at the bottom for notes. Everything a CERT Team Lead or MAG net control
operator needs to fold your report into a Community SITREP headed for the
EOC.
The back is what makes it usable by someone who’s never taken the course.
Every abbreviation on the front — S&S, FWS, COM, PWR, RD, MED, HAZ — gets
spelled out in plain language, along with what the G/Y/R and arrow marks
mean and a field-by-field guide to what actually belongs in each blank.
